Ready to join Keiki Steps? To sign up complete our online form and prepare the follow requirements:

• Current TB Test for Child(ren) and participating adult
• Updated Immunizations for Keiki (Ask your doctor for a completed Form 14)
• Birth Certificate for Keiki
